1) Apatarpana (Abstinence from food)

  • Importance
    • Abstinence from food (fasting) is usually employed for treating all types of inflammations. 
  • Indications
    • Abstinence from food is advised for the patients afflicted with excessively vitiated dosas in order to pacify them, after taking into consideration the dose involved and the strength (of the person).
  • Contraindications
    • Abstinence from food is contraindicated in the patients suffering from urdhvavata, thirst, hunger, dryness of the mouth an physical exhaustion and also in the pregnant women, aged persons, children, the weak and the timid persons.

2) Alepa (Application of Paste)
  • Early stage of inflammation and very painful ulcerative lesions should be managed by the application of pastes of the drugs of appropriate group. 
  • Application of paste is soothing; (it also) cleanses the wounds, reduces the swelling, fills up the wounds and heals it.

3) Pariseka (Spraying)
  • For the alleviation of pain of inflammation due to vitiated vata warm spraying should be done with ghrta, oil, sour gruel, meat juice or with the decoction of the drugs possessing vata-pacifying qualities. 
  • When pain is (caused by inflammation) due to vitiation of pitta, or due to trauma or poison, cold spraying should be done with milk, ghrta, honey, sugar solution, sugar-cane juice, and with the decoctions of Madhura and latex groups of drugs. 
  • (For alleviation of pain of) inflammation due to kapha warm spraying should be done with oil, (cow's) urine, caustic solutions, wine, and sukta and with the decoctions of kapha eliminating drugs.

4) Abhyanga (Anointing)
  • Anointing, applied after taking into consideration the dosas, is capable of pacifying and mobilizing them. 
  • It (anointing) has been advised to the done before local fomentation and gentle massage therapies, etc. and as an after treatment following blood- letting.

5) Svedana (Sudation, Fomentation)
  • Fomentation should be employed for the painful, indurated and hard swellings and for lesions of similar nature.

6) Vimlapana (Gentle local massage)
  • Vimlapana is indicated for fixed swellings associated with mild pain. Having given oleation and sudation therapies, a surgeon should carry out gentle local massage with bamboo reeds, the palm of the thumb.

7) Upanaha (Application of Poultices)
  • In a case of unsuppurated or partially suppurated inflammation, poultices should be applied. They resolve the unsuppurated (inflammation) and soften the partially suppurating (ones).

8) Pacana (Induction of Suppuration)
  • In case the inflammation fails to subside ,suppuration (Pacana) should be induced by appropriate drugs. 
  • The drugs mixed with Yoghurt, buttermilk, wine, vinegar, and sour gruel, should be cooked after adding oil and salt to make it in the form of an utkarika. This hot paste should be bandaged over the inflamed area covering the same with the leaves of castor oil plant. 
  • When suppuration starts beneficial and wholesome diet should be given.

9) Visravana (Blood letting)
  • For inflammations of recent origin bloodletting should be carried out to reduce pain and obviate suppuration.
  • Bloodletting is useful for the ulcerative lesions, which are hard, black, congested, painful, indurated and irregular. Ulcers caused by poisons should specifically be treated with bloodletting by the application of leeches or by incisions.

10) Sneha-Pana (Internal Oleation)
  • Oily drinks processed with appropriate drugs should be given to alleviate pain and obviate suppuration, to the patients with ulcerative lesions associated with complications.

11) Vamana (Emesis)
  • Emesis is beneficial for patients with ulcerative lesions having inflammation and excessive granulation tissue formation with adherent clots and associated with vitiated kapha.

12) Virechana (Purgation)
  • Traumatologists advocate the use of purgation (as a part of management) of chronic ulcerative lesions afflicted by vitiated vata and pitta.

13) Chedana (Excision)
  • Excision has been advised for non-suppurative, hard, fixed and necrotising soft tissue lesions.

14) Bhedana (Incision)
  • Incision is indicated for deep-seated collections of pus without any opening, for pockets of pus with (fistulous) openings and for sinuses.

15) Darana (Inducing Bursting by Local Applications)
  • Inducing spontaneous bursting by the local applications of medications is indicated in the children, the aged, the apprehensive, the debilitated, the timid, the ladies and when the lesions are situated over the vulnerable areas.
  • When the inflammatory lesion is localised and well ripened, drainage should be achieved by the drugs used for pidana (pressure).
  • When the suppurative lesion is pointing, bursting by the application of a fine paste of the drugs described for Darana or by the application of caustics.
